Memberships will auto renew unless canceled. 30-day written cancellation notice required. Memberships cannot be transferred. No refunds will be offered for unused benefits. No substitutions unless arranged with written consent. Monthly treatments cannot be banked or transferred and must be utilized within the calendar month, unless previously arranged and authorized.
